Athletes will be unable to participate in athletics until all digital forms have been completed and approved.
 
These required forms may be found on Big Teams under "Completion of These Forms is Required of Each Student" 
 
  • Physical Examination Forms - Physical Form Link: Exam Forms
    • Both Medical History and Physical Clearance Form must be completed and uploaded
  • Interim Questionnaire/Emergency Contact
  • Acknowlegement Consent to Treat and Participate Form
    • The Acknowledgement Consent to Treat and Participate Form needs to be initaled by both the parent and the student on their seperate accounts.  Parents, you will need to log into your account and initial the two places (where it says INT), do NOT have your student inital on your account.  The two places where it says student only- can ONLY be initialed on the student's account.  When the student logs into their account the lines that said "student only" will now say "INT" this is where they will initial.
 
FEES:

 

A $30 Sports Fee will be assessed to your IC account once team rosters are complete.

Fee to be paid to the school bookkeeper or pay online through Infinite Campus
