ATHLETIC REGISTRATION INFORMATION

- Physical Examination Forms - Physical Form Link: Exam Forms
- Physical Clearance Form must be completed and uploaded
- In middle school, students are required to undergo just one physical examination, which will be valid for the entire duration of their middle school years
- Physical Clearance Form must be completed and uploaded
- Medical History Questions
- These questions are no longer on a separate form that you were required to upload in the past. These questions will now be completed directly within the Arbiter platform
- Acknowledgment to VSD Athletic Policies
- Athletic Consent Form
- Concussion Acknowledgment
A $30 Sports Fee will be assessed to your IC account once team rosters are complete.
